Before you attend your first class as awaken we kindly ask that you ensure the health waiver has been completed.

We will always send this to you upon booking your first class with us. However you can also complete it by clicking on the link below.

Once completed, your data stored securely. Should anything change during your time with Awaken, it is your responsibility to advise the yoga teacher and to update your current waiver form.
